Health Information Management
At JS Global Hospital, our Health Information Management team securely maintains and protects your medical records for your care and privacy.
To request copies of your medical records, call Health Information Management at 617-499-5665, ext. 3049. Our office is open Monday to Friday, 9:00 am – 4:00 pm (closed on holidays).
How to Request Electronic or Paper Records
For electronic records not found in MyChart, complete the Request Medical Records form under the Health tab in MyChart.
For paper copies, complete and sign an Authorization Form to send and receive your records.

Accessing a Deceased Patient’s Medical Records
Under HIPAA, only a “personal representative” can access a deceased patient’s medical records. A durable power of attorney must specifically allow access after death.
Without a Durable Power of Attorney
If no such document exists, Massachusetts law requires a court-appointed personal representative of the decedent’s estate. This process involves probating the estate. For more information, refer to official probate resources.
There is no charge to send your medical records directly to another hospital, nursing home, or physician.
Personal Copy Fees
If you request personal copies, a reasonable cost-based fee may apply for producing and mailing, based on federal and state laws regarding medical record release.
Birth and Death Certificates
JS Global Hospital does not maintain copies of birth or death certificates. To request them, please contact the City Clerk’s Office.
Radiology Images
Our Health Information Management (HIM) office provides radiology reports only. For images, please contact our Film Library at [Film Library Phone Number].
Copies of Bills
For billing copies, please reach out to Patient Financial Services at [Patient Financial Services Phone Number].
Your medical record is a vital document protected by strict health privacy laws.
At JS Global Hospital, your privacy is our top priority. We strictly follow state laws for releasing protected health information (PHI), which are even stronger than federal laws. To release your PHI, you or your legal representative must complete and sign an authorization form.
